    To add a form field to a PDF, select a type of form field button from the Form tab and the cursor changes to cross hair . Drag an area in the desired position to add the form field. 

    And you can download with help of HTML. It called is Web designing.

    To create a named position for a desired destination in a PDF file using Foxit Phantom PDF, you can follow these steps:

    1. Open the PDF file in Foxit Phantom PDF.
    2. Navigate to the specific page in the document where you want to set the named position.
    3. Click on the "View" tab in the top menu bar.
    4. In the "Navigation" group, click on the "Bookmarks" dropdown arrow and select "Add/Edit Bookmarks" from the options. This will open the Bookmarks panel on the left side of the application.
    5. In the Bookmarks panel, locate the page where you want to set the named position. If there are no existing bookmarks, you can create a new one by right-clicking on the root bookmark and selecting "New Bookmark" from the context menu.
    6. Right-click on the bookmark representing the desired page and select "Properties" from the context menu.
    7. In the Bookmark Properties dialog box, navigate to the "Actions" tab.
    8. In the Actions tab, select "Go to a page in this document" from the "Select Action" dropdown menu.
    9. Under the "Settings" section, select the desired page number from the "Page" dropdown menu.
    10. In the "Zoom" section, you can specify the zoom level for the named position if desired. Leave it as "Default" for normal zoom.
